NOTE: We DO NOT take prescriptionrequests over the phone! 

Requesting Prescriptions
Requests for prescriptions should be made in writing. It is important to give the request in writing as many people know their tablets by colour or shape only and this can lead to errors in providing scripts over the phone.

You can get a prescription by: 
  • Writing to us with a list of the medications that you need with an SAE if you want it posted back to you
  • Faxing your request to us at 074 91 41563
  • Leaving in a list of the medications that you need, and letting us know which pharmacy you want it sent to
  • Asking your pharmacist to liaise directly with us about your regular medications
  • Email requests to admin@liffordhealthcentre.com
Please give us 24 hours notice!

Medical Certificates
Requests for Social Welfare Certificates (long coloured form) and/or Private Medical Certificates (short white note) can be made by phone or by calling to the centre.

The Social Welfare Cert must be collected by the person themselves (other than in exceptional circumstances). For the first episode of illness, you must consult with one of the doctors.
Social Welfare Certs will now be sent electronically to the Dept.

You need to provide the following information:
  • Your name and DOB (Date of Birth)
  • Dates of ill health
  • Nature of illness
  • The name of your regular doctor
  • PPS number
